| Description | The project "Exchange of Information within the Health Care Sector" made in cooperation between the Hospital of Aalborg and four general practicing medical centres, was one of winning project in the invitation to tender held by The Digital North Denmark.
The project will be of significant importance to senior citizens, receiving home care from the municipality of Aalborg and who need to be hospitalised or consult their doctor. On a long term basis, the remaining groups of citizens of the County of North Jutland will benefit from this, too.
With a short story of Mrs Hansen we will illustrate what it means to her that the hospital, municipality and doctors are exchanging electronic information among each other and how she will obtain better treatment, nursing and care. The goal of the electronically enabled communication is to make it easier for the involved parties to communicate, thereby making sure that
Mrs Hansen receives a faster and more efficient treatment
The information exchanged is correct
A more holistically orientated treatment and care can be administered with more information made available to the partners.
Hospitalising
Mrs Hansen is 82 years old and lives on her own in a flat in downtown Aalborg. She suffers from impaired vision so she receives cleaning assistance from the municipality of Aalborg. One day she falls over in her living room and breaks her thighbone. On her admission to hospital Mrs Hansen is entered in the electronic patient files of the hospital. The system will automatically dispatch a note to the data system of the municipality of Aalborg's Department of Care for the Elderly and Disabled containing a date of admission. The cleaning assistance Mrs Hansen was due to receive two days after her admission will be called off.
'Care' automatically sends a response to the hospital stating the local area where Mrs Hansen lives, the identity of the contact person representing the municipality related to the hospital, and what type of help she has been receiving from the municipality prior to her admission.
This exchange of information is also taking place today, however, it is done via telephone and letter correspondence, whence it is slower and not always comprehensive; further, there is a risk of misunderstandings as the receiver of the information might not always be the appropriate person.
Back Home Again
Mrs Hansen is discharged from hospital after eight days. Prior to her discharge, the hospital will electronically advise the municipality that Ms Hansen will be brought home and will need help for personal care, such as meals to be delivered, a walking aid and the district nurse to call on her for the next period of time. Now the municipality can schedule the help for Mrs Hansen prior to her return from the hospital and make everything ready and prepared for her on her return. The hospital has also dispatched a discharge summary to Mrs. Hansen's general practicing doctor.
Contact to the Doctor
A few days later the district nurse finds that Mrs Hansen needs more medication for her eyes. She orders the medication electronically at Mrs Hansen's doctor. He has already been electronically notified that Mrs Hansen receives help from the municipal department home care and aid/care.
A few months later Mrs Hansen is well enough to get to the doctor – using her walking aid - to get treatment for a blood pressure condition. Related to this session the doctor sends the district nurse an electronic message asking her to measure Mrs Hansen’s blood pressure once every two weeks and to administer medication prescribed for the blood pressure condition. |
